One man was taken to the Alfred Hospital in a serious condition with upper body injuries.
Paramedics treated another man at the scene.
Police are investigating the fight and no arrests have been made.
It’s understood the men involved were drinking at the pub before the fight broke out on the street.
The Royal Hotel bills itself as “Melbourne’s oldest and only public bar with topless barmaids”.
It is believed the men involved in the brawl were football supporters who had attended yesterday’s Queen’s Birthday clash between Melbourne and Collingwood.
More than 83,000 fans attended the game.
